Individual Support

Individual support is a personal and individualised support that is provided specifically for you, to assist you to live the life you choose with the support that you choose.

For many people with a disability, having Individual Support means being able to live in their own home and in the community with supports in place to allow independence. This is a very personal and individualised service that is tailored to suit your support needs and your lifestyle. The Individual Support program ensures all your personal support needs are met. Support may include assistance with:

· Cooking, nutrition, diet
· Dressing, grooming
· Bathing/showering
· Shaving
· Personal Hygiene
· Lifting/transferring
· Assisting with Medications
· Shopping
· Assistance with bill paying
· Assistance with meal preparations
· Light domestic duties.

Service Information
Eligibility

If you have funding in your NDIS plan for Assistance with Daily Life; Improved Relationships; Shared Living; Maintain Informal Supports or Assistance/Increased Social and Community Participation you may be able to use that funding to purchase this support service.

The Individual Support Program also provides care and support for:
· Transport Accident Commission (T.A.C.) where care and support is required for people recovering from transport accident injury
· Work Cover – where care and support is required for people following workplace accidents

 

Cost

Personal expenses – meals, drinks, entry fees etc. Travel expenses (NDIS)

Availability

Placement is available upon request and availability. Access to the regular booking cycle is available after the initial interview.
